Replacing an ACR I-Card

When To Apply

You are faced with the task of Replacing an ACR I-Card if you have been issued with an ACR I-Card and it has not expired, and one of the following has occurred:

  • You have lost your card or it has been stolen.
  • You have damaged the card. If BI damages your card you should not pay the fee.
  • The card is rejected by you or BI (incorrect entries or information printed in the card due to your fault or negligence)
  • You need to make amendments to the information on the card

You will be taken to a map of the Philippines which shows most of the offices of BI.

Clicking on any of the locations takes you to an alphabetical list of those offices. It is important to note that you then need to scroll through the list to the profile of the office nearest you, to ascertain if they have the capability to handle ACR I-Card replacements.

Documentary Requirements

These vary for each type of replacement listed in “When To Apply”. They are as follows.

Lost Or Stolen Card

  • Completed Application Form (BI FORM 2014-08-010 Rev 0)
  • Photocopy of:
    • Passport bio-page
    • The page where the visa is
    • The page with latest admission date and validity of stay.
  • Certified true copy issued by the Records Section of the Board of Commissioner (BOC)’s Order granting the visa (for internal verification purposes)
  • Letter request for re-issuance of the ACR I-Card
  • Affidavit of Loss
  • Police Report

Damaged Card

  • Completed Application Form (BI FORM 2014-08-010 Rev 0)
  • Photocopy of:
    • Passport bio-page
    • The page where the visa is
    • The page with latest admission date and validity of stay.
  • Certified true copy issued by the Records Section of the Board of Commissioner (BOC)’s Order granting the visa (for internal verification purposes)
  • Original ACR I-Card, to be returned to BI
  • Official receipt(OR) for payment for applicable fees

Incorrect Entries Or Amendments To Card

  • Completed Application Form (BI FORM 2014-08-010 Rev 0)
  • Photocopy of:
    • Passport bio-page
    • The page where the visa is
    • The page with latest admission date and validity of stay.
  • Certified true copy issued by the Records Section of the Board of Commissioner (BOC)’s Order granting the visa (for internal verification purposes)
  • Entries Regarding:
    • Name
      • Court Order (if applicable)
      • Amended/Corrected Birth Certificate
      • Affidavit of Amendment of Name/Inclusion of Name
    • Changes of Citizenship/Nationality
      • Official document proving change of citizenship/nationality
      • Affidavit of Change of Citizenship/Nationality
    • Civil Status
      • Official document proving change of civil status:
        • Marriage Certificate or Marriage Contract
        • Death Certificate
        • Annulment /Divorce Decree
      • Affidavit of Change of Civil Status
    • Address Changes
      • Certificate of Residence from the Barangay Captain
      • Photocopy of ACR I-Card
      • Affidavit of Change of Address


    The Following Applies To All Applications

    • The application form is written in the English language.
    • Your answers must be in English
    • All entries must be in CAPITAL LETTERS using English characters only. Letters like ñ, é, ü and ç are not recognized by the system. For instance, please enter names like “Muñoz” and “Sémonin” as “Munoz” and “Semonin” respectively.
    • Enter your information, make sure it can be read easily, into the appropriate spaces on the form.
    • If the information required is not applicable, please write “N/A”. Do not leave any item blank. The Bureau will not act on application forms that are incomplete and incorrectly completed.
    • You must include in your application all the documents required in the Checklist of DocumentaryRequirements (CDR).
    • Arrange all the documents required in the order as listed in the Checklist of Documentary Requirements (CDR). The Application Form should be on top.
    • Place in a legal size (8½ x 14 in.) folder. Otherwise, your application will not be accepted.
    • All sworn statements or affidavits must be original notarized.
    • All civil registry documents issued in the Philippines (i.e. Birth Certificate, Marriage Certificate, Death Certificate, etc.) must be original and issued by the National Statistics Office (NSO) of the Philippines.
    • All foreign documents must be original and authenticated by the Philippine Foreign Service Post (FSP), which has jurisdiction over the country of issuance.
    • The Department of Foreign Affairs (DFA) will need to authenticate all documents issued by a local Embassy in the Philippines.
    • If a document is written in a foreign language, you must obtain a translation into English, verified by the issuing embassy.
    • If the application is filed by an authorized representative, attach a photocopy of the Bureau of Immigration (BI) Accreditation
    • If filed by an attorney they will need to produce an Identification (ID) Certificate or an original Special Power of Attorney (SPA) for the applicant.
    • The original valid passport of the applicant must be presented to the concerned frontline officer for identification purposes. It will be returned immediately.


    The Process

    • Secure and fill out an application form and attach required documents. (As above)
    • Submit application with complete requirements.
    • The database is checked for any derogatory record.
    • Get an Order of Payment Slip (OPS).
    • Pay the required fees, and obtain Official Receipt
    • Submit a copy of Official Receipt to the appropriate Officer.
    • The Application is evaluated and approved or declined(very rarely).
    • Receive claim stub
    • When the card is ready you will be informed. Keep in contact with your local office.
    • Present your claim stub and some acceptable ID. (Passport or PH Drivers license)
    • Pick up your new card
